metabolic syndrome is associated with higher wall motion score and larger infarct size after acute myocardial infarction

چکیده
conclusions we showed that patients with metabolic syndrome have larger infarct size compared to control subjects. background infarct size is an important surrogate end point for early and late mortality after acute myocardial infarction. despite the high prevalence of metabolic syndrome in patients with atherosclerotic diseases, adequate data are still lacking regarding the extent of myocardial necrosis after acute myocardial infarction in these patients. objectives in the present study we aimed to compare myocardial infarction size in patients with metabolic syndrome to those without metabolic syndrome using peak ck-mb and cardiac troponin i (ctni) at 72 hours after the onset of symptoms. patients and methods one-hundred patients with metabolic syndrome (group i) and 100 control subjects without metabolic syndrome (group ii) who experienced acute myocardial infarction were included in the study. diagnosis of metabolic syndrome was based on the national cholesterol education program adult treatment panel iii (ncep atp iii) guidelines published in 2001. myocardial infarction size was compared between the two groups of patients using peak ck-mb and ctni level in 72 hours after the onset of symptoms. results peak ck-mb and ctni in 72 hours were found to be significantly higher in patients with metabolic syndrome compared with control subjects (both p < 0.001). patients with metabolic syndrome also had markedly higher wall motion abnormality at 72 hours after the onset of symptoms as assessed by echocardiographically-derived wall motion score index (wmsi) (p < 0.001). moreover, statistically significant relationships were found between wmsi and peak ck-mb and also ctni at 72 hours (spearman's rho = 0.56, p < 0.001 and spearman's rho = 0.5, p < 0.001; respectively). however, association between wmsi and left ventricular ejection fraction was insignificant (spearman's rho = -0.05, p = 0.46).

Metabolic Syndrome is Associated With Higher Wall Motion Score and Larger Infarct Size After Acute Myocardial Infarction
BACKGROUND Infarct size is an important surrogate end point for early and late mortality after acute myocardial infarction. متن کاملStrain echocardiography and wall motion score index predicts final infarct size in patients with non-ST-segment-elevation myocardial infarction.
BACKGROUND Infarct size is a strong predictor of mortality and major adverse cardiovascular events after myocardial infarction. Acute reperfusion therapy limits infarct size and improves survival, but its use has been confined to patients with ST-segment-elevation myocardial infarction. The purpose of this study was to assess the relationship between echocardiographic parameters of left ventric...
متن کاملInfarct Size in Awake Dogs With Acute Myocardial Infarction
Background. Short-term cholesterol feeding has been shown to affect vasomotor tone and increase infarct size in anesthetized rabbits. The purpose of the study was to determine whether acute hypercholesterolemia reduced collateral flow to ischemic myocardium and increased infarct size in the awake dog. Methods and Results. Acute myocardial infarction was produced in awake dogs by a 4-hour left a...
متن کاملLarger infarct size associated with dysglycemia at the time of ST-elevation myocardial infarction is related to later presentation
Background Patients with dysglycemia at the time of ST-elevation myocardial infarction (STEMI) have a worse prognosis. The reasons for this are not entirely clear. Admission hyperglycemia has been associated with larger infarct size. The aims of this study were to examine the relation of acute and chronic glycemic state to myocardial scar and salvage characteristics in patients with reperfused ...
متن کاملDiabetes is associated with increased and persistent myocardial edema in infarct segment post acute myocardial infarction
Methods Sixty patients were enrolled post AMI and underwent cardiac magnetic resonance on a GE Signa Excite, 1.5T scanner with a 8-channel receive coil at 48 hours and 3 weeks. T2 maps were computed from a previously validated cardiac-gated spiral imaging sequence with T2 preparations yielding TEs=2.9,24.3,88.2,184.2ms to assess myocardial edema. The T2* sequence was a multiecho acquisition wit...
متن کاملCircadian variations of infarct size in acute myocardial infarction.
BACKGROUND The circadian clock influences a number of cardiovascular (patho)physiological processes including the incidence of acute myocardial infarction. A circadian variation in infarct size has recently been shown in rodents, but there is no clinical evidence of this finding. OBJECTIVE To determine the impact of time-of-day onset of ST segment elevation myocardial infarction (STEMI) on in...
